If one is dying in the hospital in NYC, one needs a Living Will and Health Care Proxy.
If one is dying OUTside the hospital, at home, one needs a MOLST form and a Do Not Resuscitate form, usually placed by the bed of the dying person, so the EMT staff can allow the person to die at home. Without the MOLST or DNR forms a dying person may be taken to the hospital.
